Duloxetine - sold under the brand names Cymbalta , Ariclaim, Xeristar, Yentreve, Duzela; is a serotonin-norepinephrine reuptake inhibitor. It is used for major depressive disorder. It potently inhibits neuronal serotonin and norepinephrine reuptake. It is also considered a less potent inhibitor of dopamine reuptake.
Duloxetine undergoes extensive metabolism, but the major circulating metabolites do not contribute significantly to the pharmacologic activity. Main side effects are somnolence and insomnia, reported by about 10% to 20% of patients, as well as nausea (34.7%), dry mouth (22.7%), headache (20.0%) and dizziness (18.7%),.
Duloxetine and SSRIs have been shown to cause sexual side effects in some patients, both males and females.
The role of duloxetine in the treatment of various conditions has led to divergent opinions. Owing to a large number of side effects and lack of clear advantage over existing medications, some reviews have concluded that duloxetine "should not be used" for stress urinary incontinence and "currently has no place in the treatment of depression or diabetic neuropathy."
In addition, the FDA has reported on life threatening drug interactions that may be possible when co-administered with any CNS stimulants e.g. CNS stimulants may include phentermine, amphetamine, sibutramine, methylphenidate, methamphetamine, and cocaine.
